Section 17

Community Integration Services

Section 17 care coordination for adults connecting mental health needs with everyday goals.

What this support is about

Cross Roads’ Community Integration Services provide Section 17 case-management support for adults with mental health needs. The work begins with the person’s goals and brings together treatment, community resources and practical supports through assessment and individualized service planning.

Practical ways support may help

  • Coordinate treatment. Communicate with the providers involved in the person’s care and help connect with counseling, medication providers and other appropriate services.
  • Navigate housing supports. Assist with housing applications and coordinate with housing programs or landlords around reviews and other requirements.
  • Connect with food and benefits resources. Help navigate food pantries, community assistance and benefit applications or renewal reviews.
  • Prepare for provider conversations. Help a person identify concerns to discuss at medication or treatment appointments. Prescribing and treatment decisions remain with the clinician.
  • Review the care plan. Follow up on referrals and adjust coordination around changing needs, barriers and goals.
  • Link to additional support when needed. Help connect with appropriate crisis or counseling resources; those providers carry out their own assessment and response.

How Community Integration differs

Community Integration focuses on coordination, advocacy and access to resources. It is distinct from outpatient therapy, prescribing medication, direct daily-living support and emergency response. A referral to another program does not mean Cross Roads provides that program or controls its availability.

Referrals and getting started

Use the existing referral form and select Adult Section 17. Describe the mental health and everyday concerns you want help coordinating, the providers involved and the goals you want to work toward. Follow the form’s instructions for supporting information.

The team reviews the request, Section 17 requirements and current availability, and explains any further information needed. Submitting a referral does not confirm eligibility or an appointment. For help with the form, call (207) 487-7056.
