Section 13
Adult Homeless Targeted Case Management
Coordinate housing-related resources, care and practical supports when housing is unstable.
What this support is about
Adult Homeless Targeted Case Management helps adults experiencing homelessness or unstable housing identify priorities and connect with community supports. Case managers coordinate resources around housing needs, health care and everyday stability.
Practical ways support may help
- Plan around immediate priorities. Identify housing-related needs and build an individual plan for connecting with appropriate resources.
- Navigate housing resources. Help connect with shelter and housing programs and coordinate applications or referrals to community resources.
- Connect with care. Link with health and mental health providers, including counseling, medication providers and evaluations when appropriate.
- Address barriers to follow-through. Help link with transportation, benefits, food resources and other practical supports.
- Build toward stability. Coordinate vocational or educational resources and review the plan as needs change.
Housing assistance and service boundaries
This is a care-coordination service. Case management can help a person navigate housing resources; it cannot guarantee a housing placement, rental assistance, a benefit award or an appointment with another provider. Those decisions belong to the relevant housing, benefits or treatment program.
Referrals and getting started
Use the existing referral form and select Adult Homelessness TCM Section 13. Explain the current housing situation, the supports already involved and the priorities you want help addressing.
If you have no fixed address, limited phone access or concerns about leaving messages, call (207) 487-7056 to discuss a safe way to make contact and get help with the form’s required fields.
The team reviews the referral, program requirements and availability before arranging services. Submitting a request does not guarantee acceptance, housing or a start date.
Does a case manager provide therapy or transportation?
Case managers help link people with treatment and transportation resources. A connection or referral is separate from providing the treatment, ride or other service.
