Referral guidance
One referral.
A clearer next step.
For yourself, your family or someone you support.
Who can start a referral?
If you are requesting services for yourself, are a parent or guardian, or are a provider or Cross Roads employee helping someone request services, start with the existing referral form. Enter the referent information requested and describe the service need.
Open the existing referral formChoose the appropriate service
Allow the form a moment to load. Its service choices are:
- Children’s TCM Section 13 — coordination around a child’s and family’s support needs.
- Adult Homelessness TCM Section 13 — coordination for adults experiencing homelessness or unstable housing.
- Adult Section 17 — Community Integration case management for adults with mental health needs.
- Therapy Referral — mental health assessment and treatment.
For Section 28 support for your child, call the office. Section 28 is not listed in the existing general referral form.
Completing the form
Follow the form’s instructions for required fields and supporting information. It asks about preferred contact and whether voicemail is okay; give a safe way for the team to reach you. If you have no fixed address, limited phone or email access, or difficulty providing required information, call (207) 487-7056 for help.
After submission
The team reviews the request, program requirements and current availability, and explains the next step and any further information needed. A submission does not confirm eligibility, acceptance or an appointment. If records are requested, ask the team how to provide them.
Help with a referral
Call the office for help choosing a service or completing the form. Ordinary email is for general questions; keep personal health details and records out of it.
