Section 13
Children’s Targeted Case Management
Family-centered planning, advocacy and coordination of the supports around your child.
What this support is about
Children’s Targeted Case Management is a child-centered, family-driven service. Case managers work with families to understand a child’s strengths and needs, develop an individualized service plan and coordinate connections with treatment, school and community supports.
Practical ways support may help
- Start with the family’s priorities. Assess strengths, needs and existing supports, then develop an individualized service plan with goals that matter to the child and family.
- Connect services. Help families find and coordinate treatment, developmental evaluations and other appropriate resources, including referrals to outside providers.
- Navigate school supports. Support families in communicating with schools and connecting with resources that help a child participate and succeed.
- Advocate and plan transitions. Help coordinate with providers when a child moves between services or returns home from a higher level of care.
- Review progress together. Monitor whether the services in the plan remain appropriate as the child’s needs and family priorities change.
Coordination and direct treatment
A case manager helps connect the pieces of care. A referral to a therapist, evaluator or other specialist does not mean the case manager provides that treatment. The receiving provider determines its own assessment, services and availability.
Referrals and getting started
Parents, guardians and people supporting a child can use the existing referral form and select Children’s TCM Section 13. Describe the concerns, current supports and goals you want help coordinating. Follow the form’s instructions; call (207) 487-7056 if a required field or requested information is difficult to provide.
The team reviews the request, program requirements and availability, and explains any further information needed. A referral does not confirm acceptance or a service start date.
Can case management help when several providers are involved?
Coordinating school, treatment and community resources is part of case management. Bring the questions or gaps you want to address so they can be considered in service planning.
