Children’s Services

Targeted Case Management is a child-centered, family-driven, community-based service where case managers conduct comprehensive assessments that are based on a client’s strengths and needs. Case managers produce ISPs to address those needs to coordinate advocate and develop services in that plan.

From there Targeted Case Managers can monitor progress and evaluate the effectiveness and appropriateness of services provided.

Targeted Case Managers also refer to Therapy OT 65 28 specialized 28 MST FFT,  medication management, outpatient therapy, and Developmental evaluations. They also help plan discharges from higher levels of care such as Residental crisis units and Inpatient beds so that children are more likely to have success at home and to spend less time in hospitals and crisis beds.

Targeted Case Managers also support the family in navigating the school systems and help to get other supports needed for success in school.

To qualify children must have Mainecare, a mental health disorder, developmental delay, or an intellectual disability diagnosed by an ER Doctor, a psychiatrist, or a psychologist.

We meet the family where they are at and work to gain independence and success in all settings. If a child does not have an active diagnosis we have the ability to support the family in getting a diagnosis within the first 30 days however the sooner the diagnosis is received the faster the process can begin.

Homeless Adult Services

Adult targeted case management is for adults who are currently in a shelter, on the streets or living in a place that is not fit for human habitation for reasons such as no running water, electricity, or heat. Adults who have received an eviction notice or those who are “couch surfing”.

As with children’s Targeted Case Management, we have 30 days to get a proper diagnosis however the sooner we receive that diagnosis the better. Clients will then focus on their homelessness as well as getting connected to providers and supports that aim to keep adults out of shelters, off of the streets, out of jail, and out of inpatient and crisis beds.

During the time that Homelessness is being addressed clients will also be supported in gaining access to mental health supports such as therapy, medication management, and other evaluations to determine the best course of treatment.

Clients will be given assistance with linking to transportation services and also can start vocational services so that they may be able to further their education or rejoin the workforce.
