The job of an Adult Case Manager is to provide coordination of care, make referrals, and to help clients access resources. We serve adults of all ages with mental illness. For an adult to qualify for Community Integration Services they need to have Maine Care, and a specific Mental Health diagnosis like and various other Diagnosis. Adult case managers aid with submitting housing applications to Section 8, and other public housing programs. Case Managers work to coordinate with clients and landlords to find clients a safe and affordable place to live.
Adult case managers assist with finding state and charitable resources to obtain food. We do this by utilizing food pantries, and the salvation army, for emergency boxes of food., and utilize General assistance for food, clothing, and rent if needed. We assist with submitting applications to DHHS for SNAP benefits and to assist individuals in monitoring reviews to ensure clients do not lose their food supplement benefit.
Adult case managers assist by making referrals for medication management, counseling, and other outpatient services like daily living skills. Daily Living Skills is an Adult Mental Health program that offers in-home support up to 12 hours a week. The purpose of this program is to help individuals learn basic living skills such as proper hygiene, cooking, cleaning, budgeting, coping skills, taking medication, etc.
Case Workers monitor medication appointments, to assist clients in recognizing any complications they might be having so that the client may bring that information to their Providers.
Case managers monitor upcoming DHHS reviews so that clients do not lose their Maine Care benefits. Case Managers help keep client’s housing in place, by coordinating with their landlords for annual inspections or annual housing reviews.
We assist clients with mental health monitoring. When individuals are experiencing suicidal thoughts or having extreme mental health concerns, we help them to utilize Crisis and counseling services. We help coordinate with crisis workers in their evaluation of that individual. Adult Case managers watch for symptom management so that when a crisis happens we can assist with the appropriate program such as crisis or counseling services. These services assess if the client needs a crisis stabilization unit, or if their symptoms are extreme enough to require an inpatient level of care in mental health hospital such as Northern Light Acadia Hospital.
